Every January, the membership plan’s benefits reset. We do not keep credit cards on file, so you will renew your membership at your first visit of the year.

This plan is not a Qualified Health Plan under the Affordable Care Act. This is not a Medicare prescription drug plan. Our membership plan is a payment arrangement provided exclusively to help uninsured patients of our dental practice. Membership dues must be current in order to receive services and may not be combined with any other offers or discounts.
Refund Policy
Refunds will not be provided for membership dues paid under any circumstances, including failure to schedule and maintain appointments.
